Amanda received her undergraduate degree in Biology from Olivet Nazarene University in Illinois.  She graduated with honors and went directly from her undergraduate studies to the Physician Assistant Program at Baylor College of Medicine in Houston, TX, where she received her Master’s in Physician Assistant Studies.

After graduation from PA school, Amanda was commissioned into the United States Navy as a Medical Service Corps Officer and served for 3 years of active service before deciding to leave active service to start a family and finish her commitment as a Reserve Officer.  While in active service Amanda worked in Aviation Medicine, Family Medicine, and Pediatrics.

Amanda has always enjoyed serving others and is excited to continue that here at The Health Center of Southeast Texas.  Her passion is walking alongside patients to help them understand their health and enabling them to take a proactive role in their care.

Training:  I obtained a bachelor’s degree in Chemistry from the University of Science and Technology Ghana 2001. I graduated from Texas A&M University-Corpus Christi with both my bachelor’s in nursing and master of nursing degrees in 2008 and 2014.

I am licensed with the Texas Board of Nurse Examiners (TBONE) as a Family Nurse Practitioner (FNP-C) and certified with the American Association of Nurse Practitioner (AANP). I worked in family practice in Houston and Spring area before joining the elite providers of HCSET.

I Hold a Doctorate of Nursing Practice Degree (DNP) from Chatham University with a special evidenced based capstone research on improving chronic disease outcomes through group education especially in uncontrolled diabetes.

Mathew graduated with a Bachelor degree in Chemistry and then found her passion for nursing and pursued a Bachelor degree in Nursing (BSN) from University of New Delhi, India.  After migrating to the USA in 1994 she began working in various hospitals and nursing homes as an RN.  She gained ample experience in the geriatric care and neonatal intensive care (NICU) units at Laredo medical center, Laredo, Texas and at Houston northwest medical center in Houston, Texas.  During this challenging and successful career, she realized that with an advanced degree in Family practice she could serve the rural community better.  She earned a Master’s Degree in nursing specializing in Family Nursing Practice (MS-NPC) from Prairie ViewA & M University.  After obtaining her certification she joined as a Nurse Practitioner (NP) at theHealth Center of South East Texas (HCSET) in August 2015.  She enjoys working with the group of dedicated and diligent workers at HCSET and quite privileged to serve the community of Cleveland. During her time away from work she enjoys her family life with her husband and three daughters and dedicates time for community work at church.

Training: It takes a lot of professionalism and dedication to serving the community. As experienced medical professionals we may know how to treat diseases, however, being a medical provider in this community entails knowledge beyond books, sometimes a heart goes with every medical issue we deal with every single day. We can’t help but blend in with the community, share the pain and happiness of the people we serve beyond their medical problems and sometimes a little piece of our well-being goes with the prescription we write.

I started as a mid-level provider in 2014 after I finished my Master’s Degree in Nursing, specializing in Family Nursing Practice at Charles Drew University of Medicine and Science in Los Angeles, California. During my first year at the HCSET, I was studying full time for my Doctoral degree and finally awarded with Doctor of Nursing Practice by Samford University in Birmingham, Alabama. That same year I was offered an adjunct faculty position mentoring graduate level students in one of the Universities in the country.

I hold two Bachelor degrees and a Doctoral degree from my home country before I migrated to the US with my family in 2003, practiced in Labor and delivery and Surgical /Operating Room units In Brownsville, Texas. I started teaching at the nursing program and practiced as a midlevel provider in a private clinic in the same city.

Working in this community is as fulfilling as mentoring graduate level students and I am hoping to spend more years doing what I do.

Training:  Mrs. Rojas is a long time resident of Diboll, TX. Her native land is Mexico and is fluent in Spanish. She is a Family Nurse Practitioner, Board Certified by the American Academy of Nurse Practitioners, and licensed by the state of Texas to provide family centered health care to patients across the lifespan.

She began her career in healthcare as a Licensed Vocational Nurse in 1995, and proceeded to complete her Associates Degree in Nursing from Angelina College in 1996. Since obtaining her nursing degree, she worked in a local community hospital for 20 years serving in various departments that included Telemetry, Intensive Care Unit, and Stepdown Unit. She also worked as a House Supervisor, Director of Telemetry, and served in multiple leadership groups to assist in the development of new processes such as transitioning to an electronic medical administration record and a new computer system. Mrs. Rojas went on to obtain her Master of Science in Nursing degree with a track in family practice from Walden University in 2016.

Mrs. Rojas works as a Family Nurse Practitioner at the Pediatrics . Livingston Health Center of Southeast Texas location. She enjoys working with the public serving as a health professional to assist patients and their families back to health with kindness, compassion, and a listening ear.

Training: Gwen Stephens, LCSW returned to her community college, Copiah-Lincoln in Wesson, Mississippi, in 1992 after graduating with Associate of Arts degree in Secretarial Science in 1911. to start a new career. She transferred to the University of Southern Mississippi in Hattiesburg, MS the summer of 1993 after receiving a Bachelor of Arts degree in Psychology and a minor in history and foreign language studies in Spanish in 1996. ln 1997, she attended Louisiana State University in Baton Rouge, LA to take a college course in Counseling Children while also working at a drug and alcohol detox center.

ln 1998, she attended graduate school at the University of Southern Mississippi School of Social Work, earned a Master’s degree in 2000, and was licensed as a clinician in 2006. She has worked for a nursing home, the Medicaid Waiver program, as a mental health therapist in an inpatient rehabilitation center for children and adolescents, program Director of a boy’s group home, as a stress counselor for FEMA on the MS Gulf Coast after Hurricane Katrina, Program Director of a Geriatric Psychiatric Unit, group therapist for an intensive Outpatient Program for senior adults, and has had a private practice all before relocating to Texas.

After moving to Cleveland, TX, she worked for a home health agency doing home evaluations and referrals for services. ln September of 20L4, she became the first behavioral health counselor for the Health Center of SoutheastTexas. She works in three of the four clinics: in Cleveland on Monday, Thursday, and Friday, in Liberty on Tuesday, and Livingston on Wednesday.

Training: Ms. McGee received her undergraduate degree, a Major in Biology and a Minor in Chemistry, from Xavier University of Louisiana. She taught Integrated Physics and Chemistry for 3 years at Klein Forest High School before deciding to continue on with her education. Ms. McGee then attended UTSouthwestern Medical Center at Dallas’ Allied Health School where she graduated with a Master’s degree in Physician Assistant Studies.

 Upon graduation, she worked in the fields of internal medicine and weight loss at Lakeside Medical Clinic in Conroe and pediatrics at HourClinic in Houston. In her free time, Ms. McGee enjoys spending time with her children, reading, and traveling.

Ms. McGee is currently a Physician Assistant at The Health Center of Southeast Texas. She has practiced medicine her since 2009. She is from the Spring area and hopes to give back to her community by providing superior medical services to all of her patients here.

Training: Crystal was born and raised in Cleveland, Texas.  She graduated Cleveland High School then started her career in nursing as an LVN in 2000.  She spent 8 years traveling the United States while her husband served in the U.S. Air Force.  She practiced nursing in various hospitals and in every specialty for 10 years.  She has nursing experience in medical, surgical, obstetrical, pediatrics, and emergency medicine.  She finished her nursing career teaching nursing and obtained her Nurse Practitioner in 2014 from Texas Woman’s University.  She came to this medical practice from school with compassion, kindness, and a heart to serve the community.
Crystal is a wife and mother of two active boys.  In her free time she enjoys being a mommy and serving in her church.  She volunteers with children at the church, and for pre-teen camp every summer.  She loves educating, and still enjoys educating new nurses.  Crystal is excited to serve her community through this medical practice.  She sees our community as a group of people striving to grow and improve life with each other.

Training: Dr. Jasmine Sulaiman is a board-certified family physician who serves as medical director for all clinic locations since 2006. She completed her residency at Columbia University in 1999 where she was the chief resident. She completed a post-graduate training in Health Leadership and Community Medicine from Duke University and in Faculty Development from The University of North Carolina.

 She has extensive experience in primary care, emergency medicine, and medical teaching. She is a member of The American Academy of Family Physician, World Organization of Family Physicians and Texas Medical association. She currently serves as the Medical Director at The Health Center of Southeast Texas.

 Her areas of interest are preventive health care, patient education, care of chronic disease and palliative care.
