SCHEDULING AN APPOINTMENT?

We believe your health is important. We want our patients to see their healthcare provider as quickly as possible.

 With same day scheduling, there’s a great chance that you will be offered an appointment the same day you call.

Walk-in appointments may be available, but it’s better if you call to schedule in advance. We want to make sure your wait is as short as possible.

When you schedule an appointment, be prepared to give:

  • Name, address and contact information
  • Date of birth

  • Social security number

  • Health insurance or healthcare assistance information

Please bring to the appointment:

  • Photo ID

  • Proof of Insurance (card, letter)

  • List of medicines, both prescribed and over the counter, plus vitamins, herbs and supplements

    Dr. Sulaiman is a board-certified family physician who serves as medical director for all clinic locations since 2006. She completed her residency at Columbia University in 1999 where she was the chief resident. She completed a post-graduate training in Health Leadership and Community Medicine from Duke University and in Faculty Development from The University of North Carolina.

    Crystal was born and raised in Cleveland, Texas. She graduated Cleveland High School then started her career in nursing as an LVN in 2000. She spent 8 years traveling the United States while her husband served in the U.S. Air Force. She practiced nursing in various hospitals and in every specialty for 10 years. She has nursing experience in medical, surgical, obstetrical, pediatrics, and emergency medicine.

    Ms. McGee received her undergraduate degree, a Major in Biology and a Minor in Chemistry, from Xavier University of Louisiana. She taught Integrated Physics and Chemistry for 3 years at Klein Forest High School before deciding to continue on with her education. Ms. McGee then attended UTSouthwestern Medical Center at Dallas’ Allied Health School where she graduated with a Master’s degree in Physician Assistant Studies.

    arolina.Gwen Stephens, LCSW returned to her community college, Copiah-Lincoln in Wesson, Mississippi, in 1992 after graduating with Associate of Arts degree in Secretarial Science in 1911. to start a new career. She transferred to the University of Southern Mississippi in Hattiesburg, MS the summer of 1993 after receiving a Bachelor of Arts degree in Psychology and a minor in history and foreign language studies in Spanish in 1996. ln 1997, she attended Louisiana State University in Baton Rouge, LA to take a college course in Counseling Children while also working at a drug and alcohol detox center.
