DR. JASMINE SULAIMAN MD, FAAFP

Dr. Jasmine Sulaiman MD,FAAFP is the chief medical officer of HCSET and a board-certified family physician with extensive experience in primary care, chronic care management, preventive medicine, academic medicine, community medicine, and end-of-life care. Dr. Sulaiman completed her residency from Columbia University, where she was the chief resident of the program. She has been in practice since 1999 and became the chief medical officer of HCSET in 2006 when the first clinic opened the door to the community. Dr. Sulaiman has received multiple awards throughout her career, including "Texas Family Physician of the Year 2013" and "Country Doctor of the Year 2016."

Dr. Sulaiman treats all of her patients as if they are her own family. As a strong supporter of community health centers serving the medically underserved population, she believes that education, early prevention, and teamwork are the major components of a successful outcome in all illnesses. In the role of chief medical officer, Dr. Sulaiman supervises multiple mid-level providers, mentors high school students, teaches medical students and residents, and participates in several community outreach programs. The HCSET team, under her leadership, has achieved the well-recognized "Patient Centered Medical Home" status from the National Committee on Quality Assurance, a benchmark for health center quality leaders.

Crystal Halloway

CRYSTAL HOLLOWAY, N.P

Family Practice

Crystal was born and raised in Cleveland, Texas. She graduated from Cleveland High School and then started her career in nursing as an LVN in 2000. She spent eight years traveling the United States while her husband served in the U.S. Air Force. She practiced nursing in various hospitals and every specialty for ten years. She has nursing experience in medical, surgical, obstetrical, pediatrics, and emergency medicine.

Nurzy Mathew, NP

Mathew graduated with a Bachelor's degree in Chemistry and then found her passion for nursing and pursued a bachelor's degree in Nursing (BSN) from the University of New Delhi, India. After migrating to the USA in 1994, she began working in various hospitals and nursing homes as an R.N. She gained ample experience in the geriatric care and neonatal intensive care (NICU) units at Laredo Medical Center in Laredo, Texas, and Houston Northwest Medical Center in Houston, Texas.

Tracy Lee McGee, PA

Ms. McGee received her undergraduate degree, a Major in Biology and a Minor in Chemistry, from Xavier University of Louisiana. She taught Integrated Physics and Chemistry for three years at Klein Forest High School before deciding to continue with her education. Ms. McGee then attended U.T. Southwestern Medical Center at Dallas' Allied Health School, where she graduated with a Master's degree in Physician Assistant Studies.

She transferred to the University of Southern Mississippi in Hattiesburg, MS the summer of 1993 after receiving a Bachelor of Arts degree in Psychology and a minor in history and foreign language studies in Spanish in 1996. ln 1997, she attended Louisiana State University in Baton Rouge, LA, to take a college course in counseling children while also working at a drug and alcohol detox center.

Dr. Henry graduated from La Universidad Autonoma de Tamaulipas in 1980 and went on to complete his training in pediatrics at The University of Texas Health Science Center in Houston. He is board-certified in pediatrics and enjoys taking care of his patients. He takes pride in making his patients and parents feel comfortable at their visit. Dr. Henry is fluent in English and Spanish. In his spare time, he enjoys going to the gym, running, and just simply working on things around the house.

I completed my undergraduate degree at the University of Texas at Austin where I received a Bachelor's in Health Promotion with a minor in Biology. Had a change in careers and went on to pursue nursing and completed my Master's Degree at the University of Texas Health Science Center in Houston. I also completed a Post-Master's Certificate in Emergency Medicine following my Family Nurse Practitioner program. I was an ER nurse for 5 years before completing my FNP career. Throughout my medical journey, I have always strived to reduce disparities in health care and to provide comprehensive medical care to everyone. On my free time, I love to spend time with my husband and two kids. I also love to run and play soccer.

Alexa Sweeney is a board-certified family nurse practitioner. Alexa graduated with a bachelor's degree in nursing in 2010 from the University of Texas at Houston and with a Master's degree in nursing in 2015 from the University of Cincinnati. She has experience in emergency medicine, family practice, and pediatric primary care. Alexa's interests include nutrition and functional medicine. "As an NP in pediatrics, I am in a unique position to educate the entire family on the importance of proper nutrition, which plays a vital role in the prevention of common conditions, such as Type 2 diabetes, obesity, and heart disease." When not seeing patients, Alexa enjoys traveling with family and friends to places she's never been before.

Dr. Ricardo Escobar was born in San Salvador, El Salvador. He obtained his medical degree at Universidad Dr. Jose Matias Delgado in El Salvador. Dr. Escobar is bilingual (English and Spanish). He completed residency training at Case Western Reserve University where he served as Chief Resident for the year 2021/2022. He was named Resident of the Year and received Academic Excellence Award for the year 2020/2021.

Dr. Ferdous received her medical degree with honors from Sir Salimullah Medical College, a prestigious medical college in Dhaka, Bangladesh, and finished her residency in family medicine at Texas Tech University Health Sciences Center El Paso. She also received her Master of Public Health degree from the University of Texas at El Paso with Academic Excellence Award.

Milagros Aguilar is a Licensed Professional Counselor and a Licensed Chemical Dependency Counselor. She graduated from the University of Texas in San Antonio with a master’s degree in counseling. She holds a bachelor’s degree in social work from the Interamerican University in Puerto Rico. For over two decades, she has provided clients with an empathetic, safe, welcoming, and non-judgmental atmosphere in different settings within the counseling field. She is a bilingual-speaking counselor willing to offer services in English and Spanish. Milagros supports a humanistic perspective, which she uses as a foundation for diverse theories and approaches according to the client’s needs.

Monica Williams was born in Victoria, Texas, and was raised primarily in the Spring area. She obtained her bachelor’s degree in psychology with a minor in Sociology at the University of Houston and her graduate degree in Professional Counseling from Georgia State University. Monica completed her practicum and internship at Ridgeview Institute in Smyrna, Georgia working with the Woman’s Program which assisted women with dealing with issues such as trauma, depression, anxiety, mood disorders, familial, professional, and personal stress, grief, and loss, substance use/misuse, and disordered eating symptoms. She completed her clinical hours at Newport News Behavioral Health Center working with adolescents. In addition, Monica has worked in a community mental health outpatient setting and a private practice. Monica has thirteen years of experience working with children, adolescents, and adults. Monica has proudly served her country and is a Navy Veteran.

Samantha graduated from the Philadelphia College of Osteopathic Medicine with a Master of Science in Physician Assistant Studies and as a member of the Phi Alpha Honor Society. She is also a National Health Service Corps Scholar. She loves the communities around HCSET, and she strives to help all of her patients achieve better health and well-being.
